Homeless Providers with Information for DMV Non-driver Identification Card

In order to acquire and complete a Proof of Residency form, an applicant must be currently experiencing homelessness and provide identification (e.g., birth certificate, passport, social security card, etc.). If there are any difficulties that arise in obtaining a copy of their birth certificate or other forms of identification, an applicant may contact DHS or any Certified Providers for assistance.
