The District of Columbia offers medical coverage to low income residents through Medicaid, Alliance and DC Healthy Families programs. More information about these programs can be found on the DC Department of Health Care Finance website.
You must complete the Combined Application for Benefits to apply for Medical Assistance. The DHS Economic Security Administration (ESA) determines eligibility for medical benefits.
You may either fill out the application on your computer and print it or download the application and fill it out by hand.
After you complete the application, you may mail or take your application to an ESA service center. To find your nearest Service Center, view the ESA Service Center listing.
All application forms must be signed and dated and submitted to ESA (formerly known as IMA) in order to begin processing.
