Citizenship Supplemental Form
The District of Columbia has converted an identified group of childless adults from the locally funded Alliance program to federally funded Medicaid under provisions of the Patient Protection and Affordable Care Act (P.L. 111-148). As a condition of eligibility for federal Medicaid, (effective July, 1, 2006) persons applying for Medicaid must establish citizenship. To do so, customers must provide their social security number and documentation that attests to their US citizenship.
All clients that are required to submit documentation will be notified through mail.
